CARF introduces case management standards to Aging Services accreditation area
CARF International released accreditation standards for case management for organizations providing services in the aging services field. The standards are in the newly published 2012 editions of the Aging Services and CARF–CCAC standards manuals, both in electronic and print formats at www.carf.org/catalog
“Many aging services organizations offer home and community services for diverse populations that continue to reside in their own homes,” said Sue Matthiesen, managing director of CARF’s Aging Services accreditation area. “Case management accreditation provides standards for delivering high quality services that assess consumers’ individual needs, refer them to appropriate services, and arrange or coordinate their care.”
The standards for case management in aging services can be applied on CARF and CARF–CCAC surveys conducted after June 30 this year.
The new case management standards were adapted from standards applied to other areas that CARF accredits. The standards were reviewed for their applicability to the aging services field and migrated into the Aging Services and CARF–CCAC standards manuals.
Aging services organizations have an option of pursuing accreditation for their case management programs. Organizations that manage the care of the persons served in assisted living residences, long-term care settings, or continuing care retirement communities (CCRCs) are not required to add these standards to their existing accreditation.
Case management proactively coordinates, facilitates, and advocates for seamless service delivery for persons with impairments, activity limitations, and participation restrictions based on:
- Initial and ongoing assessments.
- Knowledge and awareness of care options and linkages.
- Effective and efficient use of resources.
- Individualized plans based on the needs of the persons served.
- Predicted outcomes.
- Regulatory, legislative, and financial implications.
The delivery of case management might occur in a variety of settings, such as a healthcare environment, private practice, the workplace, or the payer community.
