About Golden Pond ElderCare Strategies

For active individuals with busy jobs, travel plans, or the desire to be the loving spouse or child rather than the primary care giver, a professional geriatric care manager is the answer. We assist those who are medically frail, mobility challenged, or experiencing memory loss. We can step in and provide the time and expertise that allows family members the freedom to attend to other commitments with confidence that their loved one has coordinated, reliable care. For adult children who live out of the area, we are well suited to assure that needs are being met and that areas of concern are addressed.

Our qualified team of Geriatric Care Mangers at Golden Pond ElderCare Strategies have nursing and social work degrees. We share the desire that each senior live their best life.   

When you call on us we arrange an assessment visit. Sometimes we have an initial consultation meeting with family members, sometimes we meet with the client and the family together for the assessment. As a result of the assessment we answer the questions being asked by the client/family/POA, and offer recommendations for services, equipment, and resources. When the client/family/POA requests on-going help, we coordinate care and monitor the client. As people age, their needs change. Our care managers provide on-going monitoring and let family members/POAs know what to expect and accommodate the needs with new or more intensive services when needs change.
