GCC is an independent Care Management service, specializing in Montgomery County and NW, DC areas. Over 50 years of combined experience working in the aging field. We work personally for you to understand your unique situation and find the BEST solution to maximize each client's quality of life by providing the following services: * Consultations and Advance Planning * In-Home needs assessment (or in other settings) * Creation of a personal care plan * Coordination of high quality home care services * Evaluation of appropriate housing alternatives * Excellent referrals for all community resources * Care Management for seniors -long-term and short-term * Support for long-distance and local relatives Hope we can be of assistance to you. www.gercare.com Margi and Renee
