Job Title: Geriatric Care Manager

Care Management Group, LLC is seeking a Part-time flexible hours Geriatric Care manager to provide case management services and advocate for seniors and their families in Montgomery County and Washington D.C. Provide comprehensive assessment and care planning to eligible seniors. Provide initial and on-going care plan implementation. Monitor in-home services to ensure continue quality and appropriateness. Provide counseling to seniors and family members during periods of stress, transition and illness. Coordinate support services and assist seniors who want to move from home to an assisted living or nursing facility. Facilitate communication among family members and between client and health care providers. Required Masters Level Social Work and license. Will consider LGSW with elderly experience.
