Job Description:	Aging Network Services, a nationally known 27 year old private geriatric care management agency, is looking for a full-time geriatric care manager who has a Master’s in Social Work to visit a small caseload of older adults in the community. Duties include making a nursing home visit to help a client adjust, interviewing and supervising a home health aide, discussing the latest cognitive changes with client’s psychiatrist and setting up home care after a hospital stay. The job entails more relationship building than paperwork. We provide excellent weekly LCSW supervision. Applicants must have a car in good condition. This job is ideal for someone who is looking to join a small creative team of social work professionals. The salary is $37,000/year.
