Capital Caring, 1 of the top 10 not-for-profit hospices in the nation, seeks a full time Social Worker to join our Largo, MD team!

The Social Worker is an active and important member of the Capital Caring interdisciplinary team who provides direct and indirect care to patients and their families. The social worker is primarily responsible for providing supportive counseling (roughly 60%) and meeting care management needs (roughly 40%) of the patient and family, as directed by the plan of care. Primary duties include psychosocial assessment, care planning and implementation, providing or triaging therapeutic interventions based on assessment of needs, resource identification, supportive counseling for patients and family members, introducing and connecting family to bereavement resources.

LICENSURE:License to practice in Maryland required.

EDUCATION:MSW from school accredited by the Council on Social Work Education.

EXPERIENCE:Minimum of two years clinical experience within an interdisciplinary team model; experience in a community setting; experience working with person with chronic/terminal illness; experience with providing theoretically based clinical interventions to individuals and families.
