Responsibilities:
The Social Worker’s responsibilities include assisting the Director of Social Services in planning, organizing and coordinating medically-related social services to attain or maintain the highest practicable mental and psychosocial well-being of each resident in accordance with state and federal regulations.
Job Requirements:
•A bachelor’s degree in social work or a bachelor’s degree in a human services or related field and
•One year of supervised social work experience in a health care setting working directly with individuals.
•Strong customer service skills
•Strong commitment to resident centered care and innovative care delivery.
