Stella Maris, located in Timonium MD is currently recruiting for a Health Manager for our Senior Day Center.
Summary:
Promote a safe environment for members and caregivers in the Adult Day Care setting.

RN, CPR, and SafeServ Certification. Minimum 3 years experience.

Monday through Friday- No Holidays

Responsibilities:
- Schedules staff to ensure that an RN is present during hours of operation.

- Conducts quarterly planning conferences for each participant enrolled in the program.

- Assists with inservice education.

- Coordinates provision of other health services provided within and outside of the Center.

- Manages medical records, to include documenting all nursing services and monitoring periodically for content and completeness. Completes ADCAP system according to established guidelines and tracks and requests Physician Order Forms. Completes RE forms and incident reports.

- Develops and coordinates nursing care plan for each participant based upon physician’s orders and daily assessments.

- Provides observation, monitoring and intervention for unstable medical conditions and updates physicians of any changes in health status.

- Other duties as assigned.
