We are seeking an RN with community care experience to work directly with clients in their homes, hospitals and retirement communities advocating for quality health, residential and home care. Professionalism and teamwork await you in a premier nurse owned Care Management Company with a 23 year history of serving elders and disabled individuls in our community.The job resposibilities include comprehensive in home Assessments, Care Planning including meetings with families and professionals and Coordination activities related to client outcomes. Individual must be independent, self-motivated and able to manage relationships with families and other professionals in a collaborative manner. On call on rotating basis and local travel required.

REQUIREMENTS:
RN, BSN, Masters, Advanced Nurse preferred
Critical thinking and problem solving skills
Excellent writing skills
Experience in home care / hospice / care management
Knowledge of long term care and resources
Prefer some management / administrative experience
Keen customer service skills
Computer competency
Own vehicle for transportation
