Description
Per diem LPN and LCSW with at least five years of clinical experience, home health preferred, to do health assessments and care planning with knowledge of local resources. Must be able to work independently and travel within a designated area of West Virginia: Martinsburg, Berkely Springs, Jefferson, Ranson, Shepherdstown, Charlestown, and Hedgeville or Virginia: Charlottesville, Lynchburg, Farmville, and Danville.
