Description
Inova Health System is currently recruiting a Geriatric Care Manager for ElderLink, a public/private geriatric case management agency. Qualified candidates should have 2 years experience in case management including assessment, care planning, and service implementation and monitoring, as well as excellent communications skills. A degree in social work or a combination of education and experience in the field of geriatrics/aging services is required. The Care Manager will plan for and identify immediate, short term, and long term care needs for patient/client; and set appropriate patient/client goals and time frames for achievement. Certification in case management or eligibility for certification within the first year is preferred.
