AAA Warman Home Care is a Joint Commission accredited, private duty Home Care Agency located in Gaithersburg, MD serving the Maryland, Washington DC, and Northern Virginia communities.  We are licensed to provide non-skilled care, most specifically in regard to assistance with Activities of Daily Living.  We are currently seeking qualified applicants for a care coordinator position.


Position Overview


 


The Care Coordinator assumes the responsibility and accountability for management of the Client’s Plan of Care and the resources needed to meet the requests of the client.  The scope of the care management process involves evaluation, planning, negotiating, coordinating, documenting and monitoring options and services that meet the client’s request. The Care Coordinator is also responsible for identifying the most appropriate caregiver suited to meet the unique needs of each client. 


 


Supervision Required


 


The Care Coordinator, in conjunction with the Field RN Case Managers, supervises the day to day activity of the caregivers who provide in-home care to Clients. 


 


Essential Functions/Responsibilities


 


 


·         Evaluates client needs for care/service and makes recommendations for service


·         Identifies opportunities to improve care provided through suggestion of new or changed activities


·         Manages relationships through telephone and email communication with all members of clients’ interdisciplinary teams


·         Identifies appropriate caregiver based on evaluation of client needs


·         Communicates both verbally and in writing the needs of the client to the caregiver, Field RN, and Director of Care


·         Documents and manages client and caregiver schedules using computer scheduling software.


·         Confirms all client schedules with caregiver and client/responsible party


·         Prepares and files client care record


·         Records all client contact and all appropriate follow up


·         Evaluates client/caregiver interaction via periodic telephone survey


·         Monitors client level of care and communicates changes to Director of Care and/or Supervisory Field RN


·         Participates in agency quality improvement activities


·         Participates in rotation of occasional on call responsibilities


·         Other duties as assigned


 


 


Qualifications


 



  • Polished oral and written communication skills, including knowledge and use of email and phone etiquette

  • Effective administrative and organization skills, with strong attention to detail

  • Critical Thinking Skills and a willingness to take initiative and offer suggestions for improvement

  • Experience or education in healthcare preferred.  RN license and/or Bachelors ideal.
